You can apply for this test at our Taipei and Kaohsiung Centre.

APPLYING AT OUR CENTRE
Before you come:

1. Download application form  ( 317 KB)
2. Fill it in with your details and sign it
3. Bring it with you when you visit us

You also need to bring with you:

• One passport-sized photographs which have been taken within the last 6 months (For ID purposes, please no headwear and show ears)
• Original and photocopy of your Valid passport

The test fee is NT$1,650 for each module. We accept cash only if you submit your form by coming to our office.

APPLYING BY POST

If you want to send us your application by post:

1.Download application form  ( 317 KB)  

2.Fill it in with your details and sign it  (Please indicate either Taipei or Kaohsiung as your preferred testing centre)

3.Put the application form in an envelope along with:

• One passport-sized photographs which have been taken within the last 6 months (For ID purposes, please no headwear and show ears)
• Photocopy of your Valid passport
• the original bank transfer receipt

4.Send everything to your preferred testing centre:
Taipei Office
Kaohsiung Office
