Please deliver a  filled and signed application form accompanied by the proof of payment to any registration centre. Read carefully the conditions of registration before you sign the application form. Your signature shows approval of the conditions of registration, that is why we only accept the original application form, we cannot accept a copy and an emailed/ faxed form.
